Thank you for taking the time to nominate yourself or your colleague for an EMRA award. All materials must be received by the due date specified for the award.  The easiest and best way to do this is by using our online form.

If you would prefer, you may use the PDF Version of the form and submit all materials (Application, Letters of Intent/Recommendation, CV) by email or fax.  It is best not to send materials as you get them (i.e. application first, then your CV, then a letter of recommendation once you receive it) because the chance of your materials becoming separated and lost increases.  If at all possible, send them as one email or fax. Please do not submit duplicate applications.
